But I want to customize the fields to match the fields in the form above it. I made a different support query (#22565), which I received a reply to (#22607), but the direction I was given to “go to pages>edit the contact page and then select form right hand side under page attributes contact template and save.” did not align with choices I saw in my page attributes menu, which I listed in (#22641). The only things I can see in page attributes are: Default Template, Blog-Full Width, Blog-Left Sidebar, Blog-No Sidebar, Blog-Right Sidebar, Full Width, Gallery, Left Sidebar, No Sidebar.
If I had already set up a “Contact” page using the Free version prior to upgrading to Pro, is it possible that the old page overwrote the form template code?
Where can I locate the form template in the .zip files from the Pro download?
Once located, is there a way to individually reinstall that component?
Or, is another option to trash or rename the contact page currently existing in the project, then reinstall the theme?
Or, can you send me the code to paste into the “text” editor of the page?
Everything else has been great, and I am overall very happy with the PRO theme upgrade.
Kindly go to pages>edit any page and select under page attributes template: Full Width or left sidebar or default (which has right sidebar) for layout options.
Now for the sidebar contact form kindly go to appearance>widgets>main sidebar drag and drop a contact form shortcode and it should replace our contact form and show yours.

I recently bought the pro version of the SKT Yoga theme and I love it! There are just a few things that I cannot seem to figure out on my own.
On the contact page, there is a theme contact form in the left sidebar. I want to use my contact form in it’s place. I have played around with pages>edit contact page>select from under page attributes full width template, and I used the shortcode to add the form to the page. This resulted in a full width contact form, not really what I need.
I also played around with adding the form to the appearances -> widgets -> add contact form, but that did not do anything either.
What else can I try?
Another thing I’d like to change on the contact form side bar is the testimonials. I would like to replace this with another widget, like the blog categories widget.
And my last question when it comes to sidebars, how do I add side bars to the other pages?
Do not use shortcode of our form in pages to avoid using it. In case its appearing on sidebar you know sidebar is widgetized so you can use contact form 7 widget in sidebar to replace our form.

Your site shows coming soon so i am not able to view. On sidebar you can add your own widget. So basically add a text widget and add the testimonial shortcode. So this way it will show the testimonials but remove the blog part.
You can use the shortcodes in the lower part of the page so that the upper part of the page contains only content and thus it shows content on the homepage as well.
