I’ve figured out most of this theme except the but in template “make appointment”

I edited the existing contact form 7 to make the drop downs appear as i wanted

But when i fill out the form with a appointment date and message the email I get does not contain the field information.

this is what I get after i click submit and check my email

Subject: [your-subject]

Message Body:
[your-message]

This e-mail was sent from a contact form on Jones Vision Center (http://jonesvision.rocks)

I ended up using Ninja forms to do the same thing which works perfectly. But I sort of like the template graphics you made for this and wanted to get it to work. Any idea on why the form data is not being sent properly?

Hi,

Kindly add all field ID in message body box in the contact form mail tab.

Im not sure what you mean. Like this? It made no difference so I gather I don’t understand what you meant or where to put it or exactly how to type the ID you are referring to. Thanks.
