Contact form on Legal Expert

HI,
Can you add some instructions to your help file about the contact form in the sidebar please? I don’t want the testimonials on the sidebar under the contact from. So I installed the contact form 7 plugin and set up a new form. Copied the shortcode into a widget in the side bar but the styling is not right. I tried the code in your shortcodes:
[contactform to_email=”[email protected]” title=”Contact Form”]
but the result is like this:

I can’t see what I’m doing wrong?

Kindly use this plugin: https://wordpress.org/plugins/php-code-widget/

Drag and drop php text widget in sidebar and use contact form shortcode inside the widget.

I have installed the plugin you suggested and in the widget I pasted this showtcode:
[contactform to_email=”[email protected]” title=”Contact Form”]
but this is still what I see:
