Contact form not showing fields

Hi, my custom contact form is not showing field names. http://test.ezhvac.com/shopfk/

I’ve seen previous forums requested use of Contact Form 7, although I prefer not to as I’ve had experienced issues with that plug in (not receiving forms) on my other site.

Hi Joe,

Instead of using text widget drag and drop php text widget in sidebar and use the form shortcode.

Hi Dave,

Where can I get the contact form shortcode without using contact form 7? I was using my own code.. The titles were not showing .. and now the whole form is gone. Did you go in and edit? Not sure what happened here. Also, I cannot edit visibility of my widgets?? All of this just happened without me changing anything.

The titles are showing , thanks. still have the issue of my widgets not showing/no contact form/showing on wrong pages.

Also, how can i change fields of contact form? Will feedback be stored in wordpress or only forwarded to email?

Hi Joe,

All of your issues was resolved from our end. Kindly check and confirm.

If you want to change the contact form fields then go to inc/custom-functions.php and change the fields from there or you can use plugin Contact Form 7 and create your own form.

Hi. The contact widget is there, thank you. One issues have not been solved.
1- Visibility for widgets is no longer an option. So, the wrong sidebar widgets are displaying.
