Unable to use “Take Action” page when clicking from another page

Hello,

I am currently using SKT activism lite theme. I finished my pages, but I have a big problem when I go to other pages and cannot go to the take action page. For example, when I click my blog page, and then try to click to my “take action” button on my header, it shows error 404. I think it is because when I click the take action button, it directs me to the url of https://www.projectenv.org/blog-right-sidebar/www.projectenv.org/get-involved. How do I fix this?

Please help me ASAP.

We checked, take action button is working fine. It is going into get-involved page.
